A hearing to surrender your parental rights regarding D.J.B. and the placement of said child for adoption is scheduled for December 10, 2019 at 10:00 a.m. The Court is located at 203 W. Main Street, Room 305, Ravenna, Ohio. You are hereby notified that failure to answer for any rights to said child or object to the placement of said child for adoption by the date set forth herein shall forever bar you from making any claims to said child, and any rights which you may otherwise have to said child shall thereafter be forever terminated.
